Two would-be 49ers starters, guard Joshua Garnett and inside linebacker Malcolm Smith, likely will be out for an extensive time after being injured in recent practices.

Garnett, who has been working with the first-team offense at left guard, is dealing with a left knee that could require a “clean-up” type of surgery. He will misss time, but likely will not be lost for the season.

Smith’s injury seems more serious. He appeared to injure his chest during Saturday’s practice and was due for further tests and exams Saturday afternoon.

“Neither one of them are good news,” Shanahan said.

With Smith out, Ray-Ray Armstrong lined up next to NaVorro Bowman with the first-team defense. First-round draft pick Reuben Foster also has been playing that position, mostly with the second-string unit.

“That would be a big (injury) for us, just because we’ve been getting so many reps together, jelling together,” Bowman said.
